Summary of Duties:
The focus of the Youth and Young Adult Street (YYA) Outreach is to make connections to stable housing with tailored
services and support of the client's choice, such as health, behavioral health care, transportation, access to benefits etc.
Staff will work with clients to identify safe alternatives to unsheltered homelessness by practicing diversion and offering
transitional housing opportunities while providing support services to locate permanent stable housing. Work involves
aligning and providing housing search and supportive services to promote participant self-sufficiency, integration into
the community and maintaining permanent housing. Position is also responsible for educating potential landlords, the
public, and community partners on programs offered by the agency and the impact they have. Frequent travel
throughout the 11-county service area and to other locations in the state for meetings, workshops, training courses and
conferences as required
Essential Duties and Responsibilities: Other duties may be assigned.
• Contact homeless participants within 48 hours of receiving a referral and meet with the person experiencing
homelessness in nontraditional settings such as campsites, parks, libraries, etc.
• Practice diversion to access a safe alternative to unsheltered homelessness, and connect the participant to
shelter, if applicable.
• Complete and maintain all required housing documentation, including verification of homelessness, letters of
homelessness, release of information forms, and other applicable paperwork; process payment requests for
security deposits and ongoing rental assistance as needed.
• Assist participants in locating and securing safe and affordable housing.
• Assist participants in the development of a strength based/solution focused individualized goal and action plan
that promotes permanent housing and self-sufficiency.
• Meet with participants regularly to provide support services for housing stability.
• Complete thorough housing inspections of rental units.
• Create and maintain relationships with landlords as well as providing advocacy and mediation to develop a
workable plan to obtain or maintain participant’s housing.
• Effectively utilize multiple means of technology, including computers, cell phone, Microsoft Office products
and others as required to complete daily job-related functions.
• Maintain knowledge of program guidelines and follow them appropriately to keep compliance with the grants.
• Enter information into the Homeless Management Information System database for family/everyone served in a
timely manner.
• Attend appropriate local action committees, planning groups, and departmental meetings as required.
• Multicounty job-related travel is regularly required.
• Must have the ability to work in a team setting and independently to accomplish all required tasks.
• Provide information and referral assistance regarding available support from appropriate community programs.
• Conduct community presentations as needed to promote all assigned housing programs.

Education and Experience:
Associate’s degree in human service, Business, Education, Counseling, Social Work or related field and one (1) year
work experience in the human service field, proficiency in Microsoft Office Products, working with the economically
disadvantaged population, as well as good communication skills and the ability to establish and maintain effective
working relationships with co-workers, the community, and participants is required.
Certificates, Licenses, Registrations:
People in this position must have reliable transportation and a valid driver's license.
Language Skills:
Ability to read, analyze and interpret written materials. Write reports, business correspondence, and manuals. Present
information and respond to questions.
Mathematical Skills:
Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and
decimals.
Reasoning Ability:
Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret a variety of
instructions furnished in written, oral, diagram or schedule form.

Employees in this position work in moderate noise and often drive from one location to another and
therefore are exposed to cold and warm weather (dependent on seasons) and occasionally walk on
slippery surfaces.
Footwear: Footwear based on work and weather conditions is required, refer to Employee Handbook “Dress
Code”.
Personal Protective Equipment: No specific PPE is required for this position.
Driving: This position is required to regularly drive.
The employee shall remain free of any alcohol or nonprescribed controlled substance use in the
