REQUIREMENTS
- Minimum qualifications: Bachelor’s Degree in Social Work by accredited college/university.
- Must hold current limited or full license to practice in Michigan without conditions, limitations or restrictions.
- Two (2) years post-bachelor experience working in psychiatric setting or with mentally ill patients preferred.
- Certified Alcohol and Drug Counselor (CADC) or Development Plan working toward CADC preferred.
- Approved CPR certification
SUMMARY
- Completes psychosocial assessments, including evaluation of psychosocial functioning, events precipitating admission, identification of patient needs, limitations, and strengths.
- Complies with The Joint Commission (TJC) and Center for Medicaid and Medicare Services (CMS) standards with regards to the quality of documentation and time frames for completions of assessments and transitions of care.
- Gathers information regarding the patient – family dynamics, background, current situation, etc. from relevant sources. Maintains timely communication with patient’s family/significant relations and is a liaison between family, facility and community resources.
- Works in conjunction with the IP Therapists to provide a wrap around of services to the patient and their support system.
- Maintains healthy and effective communication with the Treatment Team and unit staff.
- Coordinates post discharge crises plan and discharge plan for follow up mental health and medical appointments. Services as an advocate for securing social needs with community supports.
- Provides education for patient and support system. As part of the continuum of care, ensures hospital discharge packets are complete (including releases or waivers) and send via email or fax to aftercare providers.
- Assists in court processes pertaining to involuntary admission status.
- Maintains up-to-date and appropriate information for patient’s records/charts.
- Participates in various facility meetings and projects as assigned; including annual facility evaluation.
- Adheres to Social Work Ethics, along with various standards and codes of practice for all administrative agencies that apply to psychiatric hospitals.
- Upholds open communication with treatment team for all patient matters.
