Duties:

• Identifies, locates, and re-engages individuals diagnosed with HIV/STDs, including those newly diagnosed, at risk of poor treatment adherence, or lost to care, to improve access to and retention in medical care.
• Provides case management, care coordination, system navigation, education, and support to help clients access treatment, community resources, and supportive services, including Ryan White Program resources.
• Conducts client outreach, interviews, and follow-up through phone calls, home visits, and community-based settings using motivational interviewing, goal setting, and other evidence-based engagement strategies.
• Builds and maintains trusting relationships with clients, families, community partners, and healthcare providers to promote positive health outcomes.
• Delivers culturally and linguistically appropriate health education on HIV/STDs, prevention, treatment adherence, behavioral health, substance use, chronic disease, parenting, and other health topics.
• Plans and participates in community outreach, health promotion, screenings, presentations, and educational events in collaboration with community organizations and healthcare partners.
• Serves as a client and community advocate by connecting individuals with appropriate medical, behavioral health, and social service resources.
• Collaborates with Sexual Health team members, healthcare providers, and community partners to coordinate services, monitor client progress, and support continuity of care.
• Maintains accurate client records, documentation, data entry, and reports in accordance with program, medical record, and grant requirements.

Education and Experience:

• High school diploma with college experience preferred.
• Preference to applicants with experience in a health related field preferably in public health dealing with high risk individuals.

Training Provided Upon Hire:

• HIV Test Counselor Certification Training (Modules I, II, III)
• STD Treatment Guidelines
• Early Intervention Services for People Living with HIV    

Special Requirements:

• Must possess and maintain a valid driver’s license with access to reliable transportation throughout the course of employment.
• Non-traditional work hours may be required on occasion. In times of a public health emergency, may be required to report for specialized assigned duties inside or outside of Berrien County.
• Will be encouraged to obtain Community Health Worker (CHW) and/or HIV Antibody Pre/Post Test Counselor certification.
• Must be willing to obtain phlebotomy training.

Required Knowledge and Skills:

• Knowledge of HIV/STDs, including transmission, prevention, affected populations, and high-risk groups, with the ability to work effectively and respectfully with diverse communities.
• Familiar with HIV/STD community resources, including social, behavioral health, and referral services.
• Excellent verbal, written, presentation, and interpersonal communication skills, with the ability to build trust and engage clients, partners, and community members.
• Awareness of locations and activities associated with increased HIV risk, and the ability to plan and conduct outreach effectively.
• Strong organizational and time management skills, with the ability to manage multiple responsibilities and accurately document services in accordance with best practices.
• Proficiency in Microsoft Office Suite and Windows-based computer applications.
• Clear understanding of client confidentiality, client rights, and compliance with the Health Insurance Portability and Accountability Act (HIPAA).
