ESSENTIAL JOB RESPONSIBILITIES:
- Provides emotional support and psychosocial counseling to patients.
- Collects factual information from the patient, family, community agencies and others to be used by treatment team staff.
- Coordinates with staff in the development, implementation and evaluation of the patient’s individual plan of service.
- Monitors services delivered to patient to ensure that individual needs are meet.
- Provides patient’s family with information regarding patients progress and treatment options.
- Screens patients for community home placement.
- May identify discharge needs and arrange for comprehensive services for patients following discharge from the hospital.
- Maintains records and prepares reports related to the work.
- Maintains patient information in confidential manner.
