Job Overview:
Zealous Home Health Care is seeking a Licensed Practical Nurse (LPN) to join our compassionate and dedicated team. As an LPN, you will be responsible for providing a variety of patient care services in patients' homes, including administering medications, monitoring vital signs, assisting with daily living activities, and supporting patients in their recovery or health maintenance plans. You will work closely with a team of registered nurses, and other healthcare professionals to ensure the best care possible for each patient.
Key Responsibilities:
- Provide nursing care in the home setting as prescribed by the physician and in accordance with agency policies and procedures.
- Administer medications and treatments as ordered by physicians, including oral medications, injections, and wound care.
- Monitor patient vital signs, including blood pressure, pulse, temperature, and respiration, and report changes to the supervising Registered Nurse or Physician.
- Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility.
- Educate patients and families on managing health conditions, medication regimens, and proper use of medical equipment.
- Perform routine physical assessments and document findings in the patient's medical record.
- Collaborate with the interdisciplinary team to develop and adjust care plans based on patient needs and progress.
- Respond to medical emergencies and provide first aid as needed.
- Assist with care coordination and communication between healthcare providers to ensure continuity of care.
- Maintain accurate patient records and reports as required by state and federal regulations.
- Ensure patient safety and comfort while promoting a respectful and supportive environment.
Qualifications:
- Licensed Practical Nurse (LPN) licensed in the state of Michigan.
- Current CPR/BLS certification.
- Previous experience in home health care, nursing home, or hospital settings is preferred.
- Ability to work independently and as part of a multidisciplinary team.
- Strong communication and interpersonal skills, with the ability to interact with patients, families, and healthcare providers.
- Knowledge of home health care regulations and best practices.

- Ability to lift, move, and support patients in a home care environment.
- Reliable transportation and a valid driver’s license.
Physical Requirements:
- Ability to stand, bend, reach, and move patients when necessary.
- Ability to lift up to 50 pounds or more with assistance.
- Comfortable in a home care setting, with various patient environments.
