TITLE: ADMISSIONS COORDINATOR
REPORTS TO: DIRECTOR OF NURSING
JOB SUMMARY:
Under the general direction of the Director of Nursing, coordinate with hospitals, nursing homes, all other agencies and families to review potential residents for admission to the Facility ensuring the best possible transition for those residents who are admitted.
PRIMARY DUTIES AND RESPONSIBILITIES:
1. Works with hospitals and nursing homes as well as family members to coordinate and review potential residents for admission to the Facility. Coordinates information on potential residents for Interdisciplinary team, makes visits to potential residents as necessary and coordinates all aspects of admissions.
2. Will be responsible for ensuring that Preadmission Screening (PAS)/ Annual Resident Review 3877/3878 are correct before admission in collaboration with Social Services.
3. Coordinates with physicians and other involved healthcare professionals to identify and clarify specific orders such as wound care, bariatric equipment needs, oxygen therapy, IV’s and any other special needs prior to a resident’s transfer.
4. Ensure a smooth transition from previous to current level of care for each resident by securing equipment, communicating with Interdisciplinary team, the resident and their family.
5. Assists residents and families with the Admission process. Communicates with family members and caretakers regarding the needs of the resident and anticipated plans.
6. Provides family tours.
7. Assists Billing Dept. in identifying appropriate responsible insurance. Verifies insurance with various insurance agencies with assistance from Admissions Clerk. Initiates insurance precertification’s when required.
8. Participates in setting up of resident chart by adding appropriate diagnoses, history and pertinent data from hospital or previous facility.
9. Is knowledgeable in Resident’s Rights and ensures resident privacy and dignity and helps ensure a safe, secure environment for all residents.
10. Completes annual in-service training requirements, maintains acceptable attendance, and dresses in accordance with Dress Code Policy.
11. Responsible for appropriate use of Facility supplies and equipment to minimize loss, waste, and fraud.
12. Maintains confidentiality of all data, including resident, employee, and facility information.
13. Maintains statistical data and compiles reports in collaboration with Director of Nursing.
14. Performs other duties participates on various committees as assigned.
JOB QUALIFICATIONS:
1. Nursing license preferred in the State of Michigan and current CPR card.
2. Marketing/business and long-term care experience preferred.
3. Supervisory skills necessary to effectively manage assigned duties.
4. Interpersonal skills necessary to work productively with all levels of healthcare personnel, effectively communicate with residents, families, and general public.
5. Mental ability to handle pressures related to meeting deadlines, completing paperwork requiring attention to detail, and dealing with concerns of staff and residents.
6. Computer and typing skills to include at a minimum data entry, Microsoft Word and Excel. Filing and organizational skills.
7. Valid, unrestricted driver’s license.
8. Certification in ICD10 coding required or required to obtain ICD10 coding within 6 months of hire date.
