General Summary:
Under the direct supervision of the Superintendent will develop and administer case plans that assure successful transitions into independent living or the next best level of care. Involved in supervising and planning, organizing work, assigning and checking other staff’s work in a team environment. Under the direction of the Superintendent, directly supervises RCW and Shift Coordinator staff. Work as a community liaison to promote the Fairview program and to work with residents to coordinate other transitions.
Employment Qualifications:
Education: Bachelors Degree in Social Work or Psychology preferred
Experience:
Three (3) to five (5) years experience working with senior citizens or the disabled in a nursing home, extended care facility or other comparable setting.
Three (3) to five (5) years experience in case management, discharge planning, substance abuse counseling, and/or mental health counseling.
Other Requirements:
Able to obtain a CPR and First Aide Certification.
Valid drivers license and ability to visit sites in the community.
Experience in computer use and ability to write and log case plans.
