Prioritizes, organizes and coordinates the acquisition of historical and current records of Senior Community Care participants (prospective and active) to meet regulations and the needs of staff. Required Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. * High School diploma or equivalent required * A minimum of one year of experience in working with the frail or elderly population. * Proficient use of Microsoft Office products. * Good listening and communication skills. Preferred Qualifications: * CNA, LPN/Associates Degree preferred * Prior experience as a Medical Records Coordinator or administrative staff person with experience in an office setting working on projects and standard workload without a lot of supervision in health care preferred. Essentials: * Record the minutes of IDT's morning meetings, and prepare agenda for the next day. * Maintain database of participant utilization, and input authorization for these services in the EMR. * Obtain discharge summaries for ED, Inpatient, and Rehab stays, and upload into the EMR. * Communicate with appropriate people as necessary for exchange of information, clarification and follow-up. * Use discretion to protect the confidentiality of information regarding employees, participants, or families. * Oversee the scanning of documents in the EMR in a manner that ensures accessibility of medical information to IDT members. * Request, obtain and make records available for IDT use for all potential enrollees.
