General Purpose of Job:
Responsible for the administrative oversight and coordination of Home Hospice. Plans and collaborates with referral sources and Care Providers for the provision and continuity of care for Home Hospice. Serves as a liaison and educator for the clinical services directly related to the Home Hospice and Home Hospice Transition programs..
Essential Duties:

- Assist in developing a vision and philosophy of care for the Home Hospice program which motivates the clinical staff to achieve operational goals and promotes referrals in the Mid-Michigan area.
- Plays a leading role in facilitating referrals from other Mid-Michigan providers by developing and nurturing key relationships in the health care community that serves the terminally ill.
- Provides leadership for the integration of the Home Hospice Care in the Hospice and Palliative Care Service line.
- Serves as the key resource and manager for all clinical issues impacting Home Hospice and Transitions/ Supportive Care Programs.
- Oversees the clinical staff practice of Home Hospice and Transitions / Supportive Care providers.
- Screens, hires, orientates and evaluates all Home Hospice Caregivers.
- Provides support to Home Hospice clinical staff. Supports effective administrative staff and policies that impact hospice clinical staff by daily oversight and as part of Managerial On Call responsibility in rotation with other management caregivers.
- Attends interdisciplinary team meetings and develops staffing patterns for both the Home Hospice and Transitional Care programs.
- Provides leadership and assists with the informed consent process.
- Counsels patients, Caregivers, and physicians through the transition to Hospice care.
- Counsels team member and assist in determining appropriate placement or level of hospice care.
- Monitors and is accountable for quality patient care as well as patient and family satisfaction.
- Assists in establishing an annual operating and capital budget for Home Hospice Care. Accountable to budgetary constraints.
- Assist in development of policies and procedures that impact the clinical practices of Home Hospice caregivers.
- Follows established HIPPA Privacy Procedures when using and/or disclosing protected health information.
- Maintains and protects patient rights under the HIPPA Privacy standard.
- Ensures that services of Home Hospice and the Transitional programs meet regulatory and accrediting guidelines.
- Serves as one of the principal spokespersons for Sparrow Hospice, educating and promoting the continuum of care within the Health System.
- Works collaboratively with the Sparrow Foundation to identify grants and assists in writing proposals for funding.
- In collaboration with the Hospice and Palliative Care teams, analyzes data, identifies opportunities for improvement, coordinates action plan and implements changes
- Participates and takes a leadership role in Hospice and Palliative Care fund raising events.
General Requirements |
• Current Michigan license to practice as a Registered Nurse. • Preferred - Hospice certification |
Work Experience |
• Minimum two years supervisory/leadership experience • Hospice and/or Home Care experience • Palliative Care experience |
Education |
• Bachelors of Science in Nursing (BSN) • Preferred - MSN |
Specialized Knowledge and Skills |
• Clinical knowledge and analytical abilities to formulate effective care planning and deliver appropriate care. • Interpersonal skills to provide effective and appropriate communication with patients, families, physicians, other Sparrow Health System Caregivers, and external agencies. • Able to demonstrate the knowledge and skill necessary to provide care based on physical, psycho/social, educational, safety, and related criteria, appropriate to the Hospice and Palliative Care patients services, as related to the principle duties and responsibilities of the position. The skills and knowledge needed to provide such care may be gained through education, training or experience. • Excellent documentation and communication skills |
