Job Description
General Purpose of Job:
The Director of Hospice and Palliative Care is responsible for the development, management, and leadership of the Sparrow Hospice and Palliative Care Program, which includes Hospice House of Mid-Michigan, community Home Hospice, and Palliative Care Services. The Director has primary responsibility for monitoring and compliance of the hospice and palliative care programs with all applicable standards, regulations and requirements. In addition, the Director serves as a liaison between hospitals, clinics, and the community for patients receiving services to ensure high quality patient care, community access to services, and
community education in end-of-life care.
Essential Duties
Provides care coordination and supervision of hospice and palliative care, assuring high quality patient care, and community access to services.
Assists in developing and takes the lead in communicating a shared vision and philosophy of care for the terminally ill throughout the continuum of Palliative Care and Hospice.
Works with leadership to provide 3-5-year strategic planning insight, highlighting areas of opportunity and impact to the SHS.
Responsible for the best practice research, project planning, proforma creation, and implementation of system-level hospice and palliative care strategies.
Develops and maintains an annual operating and capital budget for Home Hospice, Hospice House, and Palliative Care.
Identifies the need for revision of existing services or development of new services in response to changing needs of the terminally ill in the Mid-Michigan area and implements new programs to meet those needs.
Develops liaison relationships with other Mid-Michigan health care providers that serve the terminally ill.
Advocates for Hospice and Palliative Care within the acute care setting staff, in the communities and with other hospice programs.
Promotes consistent, open, two-way communication with all Hospice and Palliative Care staff.
Communicates clearly, regularly and effectively with all levels and functions to create an atmosphere of mutual respect, openness and encouragement.
Serves as an administrative resource for patients and family for Hospice and Palliative Care.
Responsible for the recruitment, selection, and development of the management team for the Home Hospice, Hospice House and the Palliative Care program.
Delegates clear responsibility, accountability, and authority to meet mutually agreed upon expectations.
Monitors quality, patient satisfaction, and financial performance of the total Hospice and Palliative Care in the communities, in the inpatient Hospice unit, and acute care setting
Provides assistance to supervisory personnel in establishing clear role identification, departmental objectives, determining staffing needs and standards of productivity, establishing policies and procedures and appropriate nursing care delivery systems with corresponding posting descriptions
Ensures that the Home Hospice, Hospice House and the Palliative Care programs meet all Federal, State, and Local Laws, regulations, and codes pertaining to health and safety.
Prepares and submits all reports required by the Michigan Department of Community Health to maintain nursing home licensure for the Hospice House.
Represent the Hospital and Health System in the community, educational institutions, and professional organizations.
Assist in development and implementation of special projects.
Ensures that the Home Hospice programs in the communities as well as the inpatient Hospice facility meet the Conditions of Participation for reimbursement by third party payers.
Serves as the principal spokespersons for Hospice and Palliative Care at professional, community, and fundraising activities.
Builds rapport and establishes credibility with the physician leadership and members, and Sparrow senior leadership.
Maintains the confidentiality of the meetings attended.
Follows established HIPPA Privacy Procedures when using and/or disclosing protected health information.
Maintains and protects patients’ rights under the HIPPA Privacy Standards.
Job Requirements
General Requirements
• Must maintain active State of MI RN licensure if a nurse
• Must maintain other professional State of MI licensure as applicable to education (such as MSW, physician)
• Must maintain active State of Michigan driver’s license
• CHPN (Certified Hospice and Palliative Care Nurse) - preferred
Work Experience
• Minimum of two years’ experience in management and supervision
• Acute care nursing experience - preferred
• Experience in Hospice, Home Hospice and/or Palliative Care - preferred
• Knowledge of Medicare and licensure regulations, standards, policies and procedures as related to Hospice and Palliative Care services - preferred
Education
• Bachelor degree in nursing, health care administration, or other health related field.
• MSN (Master's in Nursing)- preferred

• Excellent communication skills, with the ability to clearly communicate vision, mission, goals, themes, and key priorities that guide and focus the efforts of others
• Excellent prioritization skills, including the ability to coordinate and balance requirements between multiple clinical initiatives and administrative activities.
