Positions Location: Lansing, MI
General Purpose of Job:
To oversee and coordinate the clinical services directly related to the Hospice House program.
Essential Duties:
- This job description is intended to cover the minimum essential duties assigned on a regular basis. Associates may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
- Assist in developing a vision and philosophy of care for the Hospice House program, which motivates the clinical staff to achieve operational goals and promotes referrals in the mid-Michigan area.
- Provides leadership for the integration of Hospice House in the Sparrow Hospice Services.
- Serves as the first line supervisor for all clinical issues related to Hospice House.
- Supervises appropriate clinical staff practices in the Hospice House.
- Screens, hires, orientate and evaluate Hospice House nursing staff.
- Provides support to clinical staff. Supports administrative staff and policies that impact hospice clinical staff.
- Develops staffing patterns for the interdisciplinary team in Hospice House.
- Provides leadership and assists with the informed consent.
- Counsel’s patients, staff and physicians through the transition to Hospice care.
- Monitors each patient’s level of care to determine that it is appropriate as defined by the patient’s Hospice benefit.
- Oversees the documentation for all Hospice House patients.
- Monitors and is accountable for quality patient care as well as patient and family satisfaction.
- Follows established HIPAA privacy procedures when using and/or disclosing protected health information.
- Maintains and protects patients’ rights under the HIPAA Privacy standards.
- Assists in establishing an annual operating and capital budget. Accountable to budgetary constraints.
- Development of policies and procedures that impact the clinical practices of Hospice House staff.
- Ensures that services of Hospice House program meet regulatory and accrediting guidelines.
- Participates and provides leadership for staff participation in fund raising events.
General Requirements |
• Current Michigan license to practice as a Registered Nurse. • Preferred - Hospice certification |
Work Experience |
• Minimum five years Hospice experience. • Minimum two years supervisory experience. |
Education |
Bachelors of Science in Nursing (BSN) |
Specialized Knowledge and Skills |
• Excellent documentation and communication skills. • Good computer skills. • Preferred - Clinical knowledge and analytical abilities to formulate effective nursing care plans and deliver appropriate care. • Preferred - Interpersonal skills effectively and appropriately communicate with patients, families, physician, other Sparrow Regional Health System Associates, and external agencies. • Preferred - Able to demonstrate the knowledge and skill necessary to provide care based on physical, psych/social, educational, safety, and related criteria, appropriate to the Hospice patients services, as related to the principle duties and responsibilities of the position. The skills and knowledge needed to provide such care may be gained through education, training or experience. |
