The Role:
As a Homecare Physical Therapist, you’ll guide patients on their journey toward renewed strength and independence. You’ll craft personalized plans of care that meet patients where they are—physically, emotionally, and spiritually. From the first evaluation to the final triumphant stair climb, you’ll be walking alongside them, quite literally helping the lame to walk again—figuratively and sometimes very literally.
Responsibilities:
- Evaluate patients’ mobility and functional limitations in their home environment
- Develop and implement individualized plans of care with clinical excellence and creative problem-solving
- Educate patients and caregivers with clarity, patience, and encouragement
- Collaborate with the interdisciplinary team to provide holistic, coordinated care
- Document thoroughly to meet Medicare and agency guidelines (yes, even Jesus needed scribes!)
- Offer strength when progress is slow and celebrate victories—big and small
What You Bring:
- Licensed Physical Therapist (PT) in the state
- Prior home health experience preferred, but a heart to serve is essential
- Strong clinical skills and the ability to motivate without pushing beyond grace
- Excellent communication and time management
- A sense of purpose and a willingness to be a steady presence in each patient’s healing process
