District Health Deparment No. 4 is looking for a Social Worker to join our team!

• Leadership for School Wellness Program (SWP) and supervision for program staff.
• Provide direct assessment and counseling services, by appointment, to children K-12.
• Participate in individual care planning for high-risk participants.
• Provide services in the school setting, office setting and/or via tele-health.
• Provide constructive, cooperative working relationships with clients, coworkers, peers, and supervisors.
• Complete and maintain accurate, adequate, and timely records in computer software products such as Microsoft Office, Electronic Medical Records software, and similar products.
• Utilize effective communication and conflict resolution skills to serve clientele in a tactful, objective, respectful manner while maintaining privacy and HIPAA compliance.
• Directs and manages fidelity to fiscal program budgets.
• Submits reports and necessary program evaluations.
• Represents the Agency at community meetings, Board meetings, and networking alliances.

Salary:  $69,390 starting salary, increasing to $71,663 after successful probation period, plus excellent benefit package.
