Minimum Requirements
- High school diploma or equivalent required.
- Strong people & counseling skills.
- Certified Community Health Worker required or ability to acquire.
- Ability to effectively interview clients in clinic, home, and community settings, present information 1 on 1 &/or small groups to clients & community organizations.
- Ability to work with a team & clinic environment.
- Ability to utilize computer electronic medical record.
- Reliable transportation & occasional non-traditional hours.
- Experience in social work &/or public health preferred.
