SALARY RANGE: $24.01/hr---$31.21/hr LBSW pay scale
$22.69/hr---$29.50/hr LLBSW pay scale
$19.81/hr---$25.75/hr Supports and Services Coordinator pay scale

RESPONSIBILITIES: This position is under the supervision of the Program Supervisor or Clinical Director and has no direct supervisory responsibilities. This position could provide wrap around, case management and/or Supports Coordination services to a variety of clientele including adults, adolescents, children and families requesting services. Responsibilities include developing treatment plans using the person centered process, providing case management/supports coordination, and making appropriate referrals. Responsibilities also include monitoring treatment plans to assess recipient’s progress and readjusting as appropriate, carrying an active caseload, preparing case histories and needs
assessments, follow-up, and aftercare. This position will be the primary Wraparound Coordinator for Delta County.
QUALIFICATIONS: For Outpatient Services Clinician - State of Michigan Licensed Bachelors of Social Work (LBSW) or State of Michigan Limited Licensed Bachelors of Social Work (LLBSW). For Supports & Services Coordinator - Possession of a Bachelor’s Degree in a Human Services Mental Health field (Such as Psychology, Sociology, Criminal Justice, or similar nature) and one year of experience working with persons with mental illness.
