The Social Worker participates in in-home assessment, intervention, and discharge; and provides care, education, and instruction. This individual will maintain a case load of 80-100 Medicaid clients (on a monthly basis) who are pregnant or have infants through the age of one year. The position is full-time and is based at the Westside Health Center in NW Grand Rapids, but the individual must drive to clients’ homes throughout the Grand Rapids area.
Must be bilingual in English/Spanish. Must have at least a Bachelor’s degree in Social Work with the appropriate licensure in the State of Michigan (LLBSW, LBSW, LLMSW, or LMSW). Master’s degree preferred. Must be self-sufficient, independent, and be able to work effectively on a team. Should have experience in providing home-based services. At least one year of post-graduate experience working with families is strongly preferred.
The Social Worker will be responsible to:
- Provide in-home education, coordinate care, implement care plans, and manage MIHP services to pre- and post-natal women and infants up to one year of age.
- Teach and support clients in the areas of stress and depression, abuse and violence, drug/alcohol use, smoking cessation, infant growth and development, parental attachment, and other psychosocial issues.
- Make referrals to community resources to address the needs of pregnant and parenting clients.
- Complete risk-screening assessments on new clients, progress notes on all visits, and discharge summaries upon completion of MIHP services.
- Participate in interdisciplinary planning with nurses, dieticians, medical providers, and community health workers for meeting client needs and health service objectives.
- Participate in ongoing quality assurance.
