GENERAL DESCRIPTION: The Case Manager will provide coordination of the assessment, training, and job placement for individuals referred through programs funded by the local agency of the Workforce Investment Act. The Case Manager will provide case management services, including completing individual assessments, developing the vocational summary, developing the individual plan of service, conducting the transitional assessment program, assisting participants to prepare for employment, and revising the plan as needed. The Case Manager is the primary contact for the participants, serving as a mentor and advocate.
MINIMUM QUALIFICATIONS: Bachelor's Degree in Rehabilitation, Psychology, Social Work, or a related field. One year progressively responsible work experience in a relevant position. Must possess excellent organizational, written and verbal communication skills. Must have reliable transportation, good driving record and be able to drive self and others to appointments. Computer literate. NOTE: Five years of progressive responsibility in a related position will be considered in lieu of a degree.
