This position provides family preservation therapeutic services with families working as a part of a team, coordinating case management services with the Program Family Worker, Referring Worker, and other service providers.  

• Provides in-home, strength based and solution focused counseling and educational services to parents and/or children who are in the process of being reunited or have been reunited after the children have been in out of home placement.
• Provides leadership and overall plan coordination for the FRP team in developing service plans with families in collaboration with the referring worker.
• Provides leadership for the planning and provision of educational workshops, parent training, and family recreational activities.
• Provides professional individual and family therapeutic intervention sessions specific to the family’s needs a minimum of 1.5 hours per week per family and provides each family with required face time weekly as required by the contract or the Agency.
• Meets contract expectations for the number of families served based on county contracts and provides each family with required face time of 1.5 weekly hours as required by the contract or the Agency.
• Participates in Family Team Meetings, Individual Education Program Planning, Wraparound Meetings and other collaborative efforts with referring workers and other community members as needed for the benefit of the family.
• Conducts trauma screenings for all children in the family as indicated and refer family to appropriate
• services.
• Completes PFS-2, Protective Factor Survey Assessments and Scoring and incorporate Protective Factors into family goals and service plans.
• Participates in regular supervisory sessions, case reviews, team meetings and other meetings as required and trainings as required by the contract or the Agency.
• Provides oral or written testimony to the court, as required.
• Develops an emergency/safety plan with each family based on reason for referral and/or current risks to the safety and wellbeing of the children.
• Develops Infant Plan of Safe Care with each family with a substance abuse history AND a child less than 1 year of age.

• Together with the Family Worker, utilizes solution-focused interviewing techniques and positive, empowering, strength-based language in communications with the family and each other.
• Together with the Family Worker, completes documentation in a way that meets the expectations for content and timeliness as required by the contract and the Agency.  
• Supports family by providing transportation, resources, and community linkages for therapeutic,
• concrete or family needs, outcomes of assessments and screens.
• Together with the Family Worker, works as a team to engage in activities with the family that assist toward goal achievement within the treatment plan described in the Initial Service Plan, including but not limited to the education of parenting skills, domestic violence, healthy communication skills, impacts of substance abuse on children, cleaning, painting, and relocating, development of routines, chore charts, family rules etc. as needed.
• Together with the Family Worker, ensures teaching, modeling, and documentation of Infant Safe Sleep teaching and practices for all families with an infant aged less than one year.
• Together with the Family Worker, ensures  an Early On referral has been made for all families with a child aged less than three years. Referrals must be documented in writing.  
• Together with the Family Worker, completes follow-up visits a regular intervals following Family Reunification case closure.
• Participation and completion as required in initial and ongoing Family Reunification Program and Family Preservation training.
• Performs other special assignments at supervisor’s request.

• Must have knowledge and skills in the area of substance abuse and co-occurring disorders.

Education:

This position requires knowledge equivalent to that which normally would be acquired through a Master’s Degree (M.A.) in the field of human services, preferably in social work. A bachelor’s degree is allowed only if the candidate has two years of family preservation program experience

Experience:

A minimum of one year of in-home, clinical work experience in a child welfare in-home program or comparable experience. Possess knowledge and experience in working in family and child services. Background should include experience in working with multi-problem children and families.
