DaVita Kidney Care is bringing our industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called Post-Acute Services. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing unneeded burden on patients and adding avoidable costs to the health care system. Your goal is to be the 'easy button' for SNF renal patient care. Your job is to ensure all our partners experience this level of service an ultimately ensure our patients' receive a higher level of care than they have experienced previously. Post-Acute Services teammates will have access to all the capabilities and resources DaVita has to offer, and will work to quickly leverage these to develop the infrastructure and rapidly grow the new service line. Ideal teammates will have relevant experience within DaVita, get energized by a start-up type environment and improving the lives of patients, and have a track record of finding ways to GSD. Teammates must be comfortable working autonomously and have demonstrated the ability to collaborate with cross-functional resources across the Village.
What you can expect:
In partnership with the Regional Operations Manager, you'll take responsibility for the growth of DaVita's SNF dialysis service by partnering with physicians, health systems, discharge planning teams and other community resources. You will also provide service excellence by acting as a support resource for patients, hospital discharge staff, skilled nursing facility staff and medical directors to ensure smooth and timely transitions of care. Success in this role will be measured by program growth, speed of patient admissions as well as patient and partner satisfaction results.
Key Components of this role will include:
Leading Program Growth - You'll be accountable to ensure we are a partner of choice: In collaboration with the Regional Operations Manager, you'll lead the growth of the program by identifying and aggressively pursuing opportunities to build relationships in the competitive dialysis landscape. . You will be accountable for the admissions process and problem solving complex issues to achieve a high level of service excellence. Our ultimate goal is to achieve world class admissions speed and remove obstacles for nephrologists, discharge planners, and SNF admissions teams. Days will be spent creating awareness of the Post-Acute Services offering and SNF locations where our exceptional care is offered. Building relationships will be vital to this role's success. You'll be the main point of contact for physicians, health systems, discharge planning teams and other community resources.
Supporting Our Patients - Guarantee our Post-Acute programs continually achieve being the provider of choice: Service Excellence is a DaVita Core Value and you'll be the key player to manage the admissions process and deliver a soft landing for patients transitioning into a post-acute dialysis setting. Your goal will be to coordinate with hospital discharge planners, SNF admissions teams, nephrologists, and others to ensure each patient is able to receive care at their chosen SNF residence, maintain their physician patient relationship, and experience true DaVita service excellence..

You'll be constantly problem solving to ensure world class admissions speed, our goal is to admit our patients within minutes, not hours. You'll be the leader proactively identifying opportunities with clinics to ensure operational excellence and admit readiness in support of the Post-Acutes program.

Minimum qualifications
Bachelor's Degree preferred
2+ years of healthcare experience, preferably in administration or admissions
Experience working with dialysis patients a plus
Ability to travel to various locations to meet with partners (internal and external) on an as-needed basis. Includes valid driver's license in state of residence and insurable driving record.
Limited overnight travel required, up to 10% as needed
Intermediate computer skills with a variety of software packages including but not limited to Microsoft Word, Excel, PowerPoint
