Under the direction of the Manager of Senior Housing, this individual is responsible for the implementation and supervision of health services provided to senior housing residents. Job Responsibilities
Examples of Work: Coordinates and plans overall health services program at Washington Woods. Supervises staff nurses and coordinates contractual health services. Conducts pre-admission assessments to determine residence eligibility. Facilitates family conferences to ensure the resident is receiving appropriate health services. Plans and conducts health education programs. Serves as resident health advocate. Advises facility management of potential resident health eligibility issues. Acts as a community health care liaison by coordinating with home care agencies and assisting with arranging special health services needed by residents. Acts as facility contact person for discharge planning after hospitalization. Identifies the need for and administers proper infection control procedures. Performs other duties and responsibilities as assigned.

Required Knowledge and Abilities
Qualifications -
Required Experience and Training:
Requires current Michigan Registered Nurse License with minimum three years practical experience in providing health care services to the elderly. Background in home health care, assisted living or long term care desirable. Bachelor’s degree in nursing preferred.
Knowledge, Skills and Abilities:
Self motivated. Proven medical/gerontology assessment skills; thorough knowledge of community resources. Ability to interact with elderly residents, their families and the medical community. Ability to coordinate programs and delegate responsibility. Excellent written and oral communication skills; familiar with e-mail and word processing software; ability to set priorities and make decisions based on sound medical knowledge.
Additional Requirements:
Possession of a valid motor vehicle operator’s license issued by the State of Michigan with a good driving record.

Pay Range $32.33 - $37.72 per hour
