The Director of Value Transformation and Patient Engagement (VTPE) is responsible for ensuring the organization is prepared for value-based reimbursement and population health initiatives. The Director ensures the organization is structured to achieve clinical outcomes as defined by stakeholders and 3rd party payers. The Director supports operations in the training and guiding of both clinical and non-clinical staff ensuring all have a full understanding on the importance of achieving quality outcomes for patients. The Director develops and maintains effective methods of engaging and motivating patients to participate in their ongoing care through the implementation of an active Community Health Worker team, digital front door, various media applications, direct mailers, and other such strategies to support the organization and its clinical teams in achieving quality outcomes and reducing the total cost of care. Frequent local, regional and National travel will be required.
Bachelor's degree in healthcare-related field or a combination of education and experience
Preferred Education:
Master's degree in Healthcare Administration, Nursing, Business Administration, Social Work, or related field
Experience and Skills
Required:
Experience in healthcare quality improvement program implementation and performance improvement activities
Experience in developing or partnering with teams designing safe, high quality workflows.
Project management experience.
Knowledge of advanced principles of PCMH/ NCQA, HEDIS, MIHIN, and UDS Reporting
Proficiency in use of Microsoft Office Suite, including Excel and Power Point
Experience creating and managing work plans and time-sensitive deliverables and deadlines
Preferred:
Five plus years of healthcare management experience preferably leading quality improvement initiatives.
Experience with healthcare clinical and non-clinical operations management
Experience with regular use of panel management methodologies and strategies
Experience in grant/plan writing, management, dissemination, and reporting
Experience with healthcare data analytics
Experience working in a Federally Qualified Health Center or non-profit healthcare organization
Experience public speaking and building community relations, including developing presentations and other verbal, or written professional documents and communications

Physical Demands:
Regular sitting, standing, and walking for prolonged periods of time.
Occasional lifting up to 20 pounds.
Requires continuous performance of detailed tasks, interruptions, and changing work priorities.
Occasionally subject to long, irregular hours.
Repetitive motion from performing daily tasks including manual dexterity particularly related to computer usage.
Visual acuity including depth perception, ocular focus, close vision, color vision.
Adequate hearing acuity and verbal communication to perform daily work.
Ability to adjust physically and emotionally to a fast-paced work environment.
Working Conditions:
Frequent regional travel.
Occasional national travel for meetings, conferences, and seminars
Frequent works without direct supervision but also as part of a team
Infrequent potential exposure to infections and contagious diseases in clinic environment.
Frequent use of PPE, particularly while present in clinical environment.
Environment may contain housekeeping/cleaning agents/chemicals, electrical/mechanical/power equipment, and biohazards and/or radioactive substances.
