Coordinate with the Care Connect, UMMG Leadership to support evidence-based care by calling patients who have been identified with a clinical need, scheduling when appropriate, and billing when appropriate for services. Participate in scheduling initiatives and other projects to support the Care Connect team.
PRIMARY RESPONSIBILITIES
- Supporting current clinic infrastructure for quality performance by helping keep teams informed of performance on relevant metrics.
- Support transitions of care efforts through scheduling patients with the PCP within the appropriate amount of time.
- Support financial sustainability of Care Connect and UMMG through supporting the transitions of care, Behavioral Health Collaborative Care, and Care Coordination billing processes.
- Support Care Connect with referral work queue management.
- Other ad hoc responsibilities as dictated by UMMG.
- Calling patients on various lists / reports to bring them back into the clinic either with Care Management or PCP, which will include but may not be limited to:
- Following a centrally developed and consistent processes for outreach for gaps in care, assisting with internal quality measures and meeting payer quality targets.
- Working the Priority Outreach Report and trying to get the high priority patients scheduled with Care Management.
- Supporting the rest of the care management team with the Longitudinal CM report, or future iterations of any such report that would describe those patients who are ‘lost to follow up’.
- Working a “Missed patients” (i.e. payer attributed but not seen in at least a year) list of patients, trying to get these individuals scheduled with their PCP.
- Calling all BCBSM Commercial PCP attributed patients (both by payer attribution and MM attribution) after visits to the Emergency Department to provide scripted education around alternate resources to the patient, such as Video Visits, alternative hours to the Primary Care, etc.
- Billing for any billable services – new BCBSM guidelines allow for MAs to bill for the 98968 (5-10 minute phone call), 99487, and 99489. MAs would selectively provide outreach to and bill the BCBSM Commercial plans.

- Provide quality service based on customer expectations and needs.

- High School diploma or equivalent.
- Graduation from a Medical Assistant program or an equivalent combination of education and 3-4 years of experience.
- Experience in panel management including provider registry data and exception reports.
- Excellent attendance record and punctuality.
- Must adhere to a high standard of personal and professional conduct.
- Demonstrate excellent written and oral communication skills.
- Must demonstrate the ability to interact with a diverse population.
- Must be an active team player with the ability to work independently.
- Possess strong organization and prioritization skills.
- Adhere to high standards of teamwork by demonstrating adaptability, flexibility, and consistently sharing information and resources with others.
- Demonstrates the ability to multi-task in a fast paced environment.
- Proficient in Microsoft Word and Excel.
- Associates degree in the healthcare industry.
- Experience working in a complex health care setting, scheduling patient appointments.
- Basic knowledge of medical terminology.
- 3-5 years’ experience in a primary care clinic or Customer Service environment.
- Familiarity with MiChart.
- Knowledge of University policies and procedures.
