ENTRY REQUIREMENTS
Graduate of an accredited School of Registered Nursing and currently registered in good standing with the State of Michigan.
A BSN, or working toward completion of, MSN preferred.
Minimum of 4 years experience in an acute care setting. Must demonstrate competency in stroke care nursing.
Demonstrated leadership in process improvement and project management preferred.
Requires strong organizational, time-management, problem-solving, independent thinking, judgment, prioritization, multi-tasking, and attention-to-detail skills. Requires self-direction and ability to concentrate while being subject to multiple interruptions and changing work priorities.
Requires computer database, graphic, and statistical analysis skills, as well as internet skills and word-processing skills. Working knowledge of data collection and analysis required.
Requires highly developed written, oral, and presentation communication skills; an ability to communicate with internal and external customers; employing a high level of tact and diplomacy; must possess strong consensus building skills.
Demonstrates knowledge of confidentiality as it pertains to HIPAA and the handling of information sensitive to Munson Healthcare.
ORGANIZATION
Works with the Stroke Team to coordinate the operations of MMC's Stroke Program for the Stroke Disease Specific Certification and designation of Primary Stroke Center.
This position reports directly to the Stroke CNS and Unit Manager of the A7 Stroke Unit. As applicable, communicates with the VP of Patient Care Services, and the VP for Regional System Development.

- QUALITY:
Concurrent chart review Develop presentations for reporting quality data Initiate quality improvement projects Gathering of data for analysis and review Works closely with the physician Stroke Medical Director, as needed Co-chairs the Stroke Interdisciplinary Team meetings with the Stroke CNS and Physician Champions, assisting team in defining direction, goals and action plans to continue to improve care of stroke patients, and compliance with stroke outcomes Data collection including chart abstraction Data entry Work cooperatively with other stroke leads throughout Northern Michigan Stroke Initiative as the MMC point of contact with the Stroke CNS. Work cooperatively with the performance improvement structure to align Stroke Certification and designation as Primary Stroke Center in a way that will support the achievement of accreditation from regulatory agencies.
CLINICAL:
Participate in the in-patient rounds for stroke Inpatient management, working through medical staff, to assure appropriate diagnostic tests are ordered Work with appropriate staff to complete documentation as necessary Responsible to propose and design computer changes necessary to help with compliance with stroke measures Collaborate with clinical documentation specialists and Stroke Team to help with appropriate documentation to optimize coding.
STAFF EDUCATION: Acts as a stroke expert and resource to the professional staff, personally providing stroke education to physicians, nursing, allied health personal/EMS Identified as responsible person for the development and implementation of a staff stroke education plan for the hospital Responsible for educating staff on the results of stroke metrics Serves as a role model for staff
PATIENT EDUCATION: Develops and/or selects stroke education materials used for patient education Ensures that patient educational materials are reviewed for health literacy and cultural competence Not individually responsible for providing stroke education/counseling but reinforces education given by others.
