Description
As directed by the Nurse Manager, the Case Manger RN manages patients through the continuum of care, promoting the delivery of quality and cost-effective health services. Assesses, plans and organizes the discharge plan in conjunction with the needs and abilities of the patient and family. The Case Manager RN also assists the Nurse Manager with day to day coordination of patient care, education, care planning and goal setting, working collaboratively with the inter-disciplinary team and medical director.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Responsible for coordinating the care for patients and identifies when alternate levels or care should be pursued in collaboration with care team members and attending physician. Conducts concurrent review/assessment of the patient's treatment plan/progress and communicates the results to appropriate care team members. Confers with care team members to identify and update changes in patient status. Determines and suggests alternative strategies.
Assesses and identifies patient and families for transition planning needs. Develops and implements the transition plan on all patients, facilitating the process of discharge planning to expedite a successful transition of care. Initiates referrals to other professional health care services for evaluation following admission review as well as on a daily basis or as necessary.
Assesses educational needs of the patient and their family and provides appropriate resources/referrals, Works collaboratively with care team members to ensure that patient/family educational needs are achieved and documented.
Assists unit nursing staff with daily coordination of patient care, care planning and goal setting.
Assesses daily staffing needs, redistributing team members as unit needs change, in collaboration with the Nurse Manager.
Provides immediate "troubleshooting" for patient care issues and follow up as indicated.
Assists with orientation of new Colleagues, acts as clinical resource, and oversees data collection for process improvement activities to sustain unit quality of care and continual improvement of work processes. Performs other2nsistent with purpose of job as directed.
Qualifications
Graduate of an accredited school of nursing.
Licensed Registered Nurse in the State of Michigan.
Discharge planning expertise normally acquired through 2 years experience in a hospital acute care setting or healthcare agencies.
