JOB DUTIES:
The following description of job responsibilities and standards is intended to reflect the major responsibilities and essential duties of the job but is not intended to describe minor duties or other responsibilities as may be assigned from time to time:
Serves as a clinical consultant to nursing staff in the assessment, planning, implementation and evaluation of resident care.
Leads resident care plan meetings - and assures individuality and appropriateness of problem statements, goals and interventions.
Collaborates with physicians and other health care professionals in effectively communicating resident care concerns and related issues.
Demonstrates leadership skills required for optimum resident care and efficient operation of assigned unit.
Assumes a leadership role in critical demanding or difficult resident, family and unit situations.
Demonstrates accountability for own practices as evidenced by follow-though or resident/employee concerns and unusual occurrences.
Assists in the evaluation of clinical proficiency of nursing staff on designated wing and collaborates with supervisor inannual evaluation of nursing staff.
Supports quality improvement by participation in quality assessment and improvement monitoring - which includes but is not limited to:
Restorative programs.
R.O.M. and ambulation programs.
Restraint utilization and justification.
Bowel and bladder management and training.
Assessment and documentation.
Appropriate use of medications including documentation of diagnosis
Infection Control Surveillance and Audits
Collaborates with physicians to assure medical orders for residents are safe, individualized and comprehensive.
Assures follow-up (in timely manner) on:
Physician - Pharmacist communications.
Pharmacist - Nurse communications.Acts as a resource to staff in administration of more complex medications and I.V. fluids according to policy and procedure.
Assures all treatments for designated units are provided according to policy and procedure and serves as resource in evaluating outcomes.

Demonstrates proper utilization and operation of-routine and more complex equipment required for resident care.
Monitors the protection of residents/staff from environmental hazards by promoting the adherence to safety and infection control policies.
Assures infection control logs are monitored according to policy and procedure. Will document infection control reports for both residents and staff.
Demonstrates dependability by completing commitments within requested time-frames.
Recognizes when others are in need of assistance and consistently offers to help when own work load permitsor finds someone who can.
Projects a positive attitudeto all individuals as evidenced by direct observation.
Meets all other job requirements as defined in licensed personnel - RN and LPN job descriptions.
Will assist Nursing Leadership with pre-admission screening including acting as liaison out of the facility for review.
Will track quality indicators and report findings quarterly to Quality Assurance.
Maintain the confidentiality of all resident care information.
Monitor nursing care to assure that all residents are treated fairly, andwithkindness,dignity,andrespect.
Monitors to ensure that all nursing care is provided in privacyandthatnursing personnel knock before entering the resident's room.
Review complaints and grievances made by the resident and make a written/oral report to the Administrator indicating what action(s) were taken to resolve the complaint or grievance.
Will assist in reviewing admission paperwork and approximating potential admissions and working with transferring caregiver to assure needed information is obtained.
Registered Nursing Staff will be responsible for starting intravenous therapy and performing phlebotomy as ordered by the physician.
Must be CPR trained for the Health Care Provider level (including AED). Must be able to maintain CPR training or be subject to job loss.
Coordinate and participate in the development of written preliminary and comprehensive assessments of the nursing needs of each resident, which addresses specific MDS trigger and RAP substantiation.
Participates in the development of a written plan of care for each resident that identifies the problems/needs of the resident, indicates the care to be given, and goals to be accomplished.
Encourage the resident and family to participate in the development and review of the resident's care plan.
Review resident care plans for appropriate resident goals, problems, approaches and revisions based on nursing needs
Reviews nurses notes and make observations to determine if the care plan is being followed.
Qualifications
JOB SUMMARY:
Provides clinical leadership in the day-to-day operations of the unit. Acts as a resource for those who assess, plan, implement and evaluate resident needs and nursing care. Demonstrates professional development / competency. Oversees and directs all aspects of resident care and employee relations supporting RN Nurse Manager.
KNOWLEDGE, SKILLS, AND ABILITIES:
Education: Registered Nurse
Experience: Two years clinical experience in geriatric population
Licensure: Current license in the State of Michigan as RN
Skills: Nursing skills as defined in the laws governing the practice of nursing in Michigan. Excellent interpersonal, communication,
negotiation, organizational, leadership, analytical, critical thinking, and time management skills. Must be able to communicate verbally
and in writing, clearly, concisely, and accurately in English.
Prefer background experience in supervision, nursing, administration, restorative services, etc.
Ability to supervise and train subordinate staff and show communication skills.
Ability to relate effectively as spokesperson for the facility to family members during times of crisis or complaint and follow facility policies and guidelines pertinent to the situation at hand.
