This Position is located in the Home Based Primary Care (HBPC) Physical Therapy division of the Physical Medicine and Rehabilitation Service (PM&RS) at the Aleda E. Lutz VA Medical Center. This position is responsible for the Physical therapy program for the Home Based Primary Care (HBPC). This position is also responsible for occasional covering for the HBPC Physical therapy programs at the other locations as needed. The Physical Therapist will adhere to all safety standards related to Home care. The incumbent must possess a valid driver's license as frequent travel is required with a catchment area often greater than 100 miles. Duties include but not limited to: Receives the patient's referral from an appropriate physician or primary care provider and schedules the patient for evaluation and treatment in a timely fashion; chart review will allow an opportunity to assess appropriateness of the referral and possible contraindications for treatment. This position may be responsible for sole PT participation in the Patient Multidisciplinary Treatment Team. Incumbent will demonstrate ability to make autonomous clinical decisions in a Direct Access environment. This includes ability to independently evaluate and treat patients who seek physical therapy services. Evaluation includes a medical history and performing and interpreting of the appropriate tests and measurements of the patient's condition for use in preparing the treatment plan, modifying treatment, evaluating results of treatment. Tests are frequently complex and may need to be adapted because of the patient's multiple physical disabilities or present medical condition. Incumbent will demonstrate skills in performing examinations and evaluations of individuals who have or may develop impairments, activity limitations, and participation restrictions related to conditions of the musculoskeletal, neuromuscular, cardiovascular, pulmonary, and/or integumentary systems while considering the effects attributable to unique psychosocial and environmental factors. Independently utilizes evaluation data to implement comprehensive patient treatment plans, involving patient/family/caregiver/HBPC staff in goal-setting process if feasible. Incumbent will demonstrate ability to establish a diagnosis within the scope of Physical Therapy and identify the appropriate rehabilitation intervention, including referral to another provider for further consultation as clinically indicated. Uses judgment and special skills in the application of specific treatment modalities to meet the previously assessed needs of patients assigned. These may include, but are not be limited to: heat, cold, light, water, sound, electricity, and mobilization. Incumbent must coordinate and employ multiple treatment approaches simultaneously to accomplish treatment objectives. Independently modifies treatment plan and treat patients according to evaluative findings on an on-going basis to better meet short and long-term goals. Education AND Experience: Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist. NOTE: Individuals must be a graduate of a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited college or university. Preferred Experience: Experience in home care physical therapy and two to three years of physical therapist experience. Examples of GS-11 experience are but not limited to; providing assessment and treatment intervention.
