Summary: Plans, coordinates and provides total nursing care for our skilled nursing facility residents and provides supervision and guidance to clinical staff members in relation to wound care. Scope of work may be modified by state specific rules under the Nurse Practice Act. Qualifications: Education: * Graduate of accredited LPN/LVN School of Nursing Licenses/Certification: * Valid License Practical Nursing (LPN) license in the state employed. * Valid CPR certification. Experience: * Two year of nursing experience preferred. Essential Functions: * Assesses, plans, implements, and evaluates the ostomy care for patients on units * Provides advanced clinical guidance to physicians and nursing staff for complex wound drainage, drainage confinement, skin protection, and the prevention of pressure necrosis (skin deterioration and skin loss) * Develops and conducts educational programs for staff * Determines appropriate nursing interventions for identified patient problems, symptoms, and teaching needs * Develops a patient care plan and determines the equipment, materials, and resources needed to successfully implement the care plan * Selects the stoma (opening) site on the patient prior to surgery, based on the patient's anatomy, activity level, and lifestyle * Performs related responsibilities as required. Knowledge/Skills/Abilities: * Skilled in directing and motivating the workforce. * Ability to work cooperatively as a member of a team. * Ability to communicate effectively with residents and their family members, and at all levels of the organization. * Ability to react decisively and quickly in emergency situations. * Knowledge of training techniques for clinical staff. * Ability to maintain confidentiality.
