Description
Organizes, files, and retrieves patient medical records. Files various medical documentation, including patient notes, radiology reports, and lab results. Performs related clerical duties. May be responsible for clerical duties related to patient admission/discharge. Works with both paper-based systems and electronic medical records (EMR). Ensures medical records are maintained in a manner compliant with ethical, legal and regulatory requirements of the medical services system.
Responsibilities:
Organize and evaluate medical records for completeness and accuracy.
Close and file discharged patients' medical charts. Review charts of discharged patients for deficient information and ensure compliance for correction of deficient documentation.
Generate and prepare/assemble medical records.
Pull and route records as requested to appropriate personnel or department.
Pull charts as needed for special audits and peer review.
Create reports when required.
Prepare copy of records when presented with properly completed medical release forms.
Perform a variety of administrative duties including but not limited to: answering phones; faxing and filing of confidential documents; and basic Internet and email utilization.
Maintain effective communication with others, both inside and outside the department, to give or obtain needed information or coordinate activities of patients (e.g., doctors' appointments).
Update job knowledge by participating in educational opportunities.
Perform other related duties as required and directed.
