Description
The Case Manager (RN) serves as the primary care nurse for a caseload of patients and provides case management, clinical care, and is responsible for coordinating all disciplines involved in providing quality, cost-effective care through direct care and supervision under the direction of the physician. The Case Manager (RN) generally does the initial assessment and mutually establishes the plan of care with the patient and family, under the direction of the physician, establishing outcomes to be achieved within the episode of care. The Case Manager (RN) periodically reviews the plan of care and provides follow up and supervision of care. The Registered Nurse/Case Manager has accountability for ensuring compliance with federal, state, and local laws and regulations, as well as accrediting organization and professional nursing standards.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Admits patients to service in accordance with admission criteria, ensuring insurance eligibility requirements and conditions of participation are met.
Completes Comprehensive OASIS assessments in accordance with standards of accuracy and timeliness, at admission, recertification, post-hospital, and discharge to establish evaluation of the patient's physical and psychosocial needs and problem identification for care planning and revision of the plan of care.
Develops an individualized plan of care and initiates services in collaboration with the patient, and family, under the direction of the patient's physician.
Coordinates patient care and services from admission to discharge through effective clinical conferences, communication, scheduling and documentation.
Confers with the patient's physician, with the clinical manager, and all disciplines providing care to the patient to ensure that the patient receives care that is medically necessary for meeting specified outcomes within the episode of care.
Qualifications
Graduate of an accredited school of nursing.
Current Registered Nurse license in the State of Michigan.
Two years of recent clinical experience, preferably in home health care.
Criminal background check, which demonstrates no convictions that conflict with established standards of conduct, and norms of professional patient care.
