Wexford/Crawford PHO is hiring a School and Community Based Mental Health Provider - Complex Care Coordinator. Our organization exists to improve the health of the people of northern Michigan and we are currently partnering with the Health Endowment Fund & the Wexford-Missaukee ISD to support the integration of behavioral health services within school settings.
Minimum Level of Education & Experience Required:
Master's degree in Human Services field (MSW or MA) with license as a mental health professional in the State of Michigan. Fully licensed preferred, limited license considered with expectation to complete and meet all requirements for full licensure.
Experience in providing mental health services with youth and family in outpatient; crisis; school or home based services. Demonstrated knowledge and proficiency in best practices of mental health treatment. Knowledge of Adverse Childhood Experiences (ACEs) and standardized assessment tools such as depression or anxiety screening. Ability to develop an individualized plan of care and to provide individual, group, and/or family interventions based on identified need in the school or community setting. Collaborates with school personnel, client/family, community providers, healthcare providers, and members of the PHO behavioral health team.
Ability to demonstrate strong customer service, public speaking, and writing skills in a fast paced and changing environment is required. Must be self-motivated and have the ability to work independently as well as a team player to accomplish goals.
