You will provide routine in-home care per a care plan and under the direct supervision of a nurse. Responsibilities: • Provide routine individualized care including but not limited to medication reminders, bathing/dressing, meal preparation, basic wound care, etc. • Monitor and report changes in patient health status • Teach family members ways to care for patients • Transport patients to appointments as well as run errands Qualifications: • Experience in home health or other health related fields (preferred but not required) • Positive and patient demeanor • Ability to build rapport with patients • Ability to handle physical workload • Excellent written and verbal communication skills • Ability to re-direct/alleviate patient behaviors Preferred: • TBI experience • CPR/TB Applications may be submitted to: 10281.axiscare.com
