Home Health offers services to patients requiring skilled care in the home environment for diagnoses related to a variety of medical conditions such as: cardiac, pulmonary, psychiatric, wound and ostomy, orthopedic, infusion, maternal and child services. Staff work as part of an interdisciplinary team dedicated to providing care, patient education and services in collaboration with the patient's physician.
The RN is responsible for the provision and coordination of patient centered care. The RN coordinates care to achieve desired outcomes, participates in patient and family teaching and provides leadership by delegating and working cooperatively and collaboratively with ancillary nursing and other members of the healthcare team in maintaining standards for professional nursing practice and professional nursing performance in the clinical setting. The RN is also accountable to practice in accordance with the Michigan Public Health Code Act 368 of 1978, Article 15 (Health Occupations), Part 172 (Nursing). The RN must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in his/her area.
** Sign on Bonus to be paid in two increments - $1,250 upon hire and $1,250 after 12 months of employment
Minimum Education/Training: Graduate of accredited Registered Nursing Program. Effective January, 2016, any newly hired RN without a BSN will be required to obtain a BSN within 5 years of hire. Exceptions will be made for equivalent experience with Director approval.
Registration/Certification or License Required: RN - Licensed to practice in the State of Michigan (or completed by end of orientation). Current BLS for the Healthcare Provider Certification (or completed within 60 days of hire). For ICU/TU, Cardiac Cath Lab, Emergency, Urgent Care, PACU, Surgery Center, Endoscopy: ACLS Certification or completed within one year of hire. For Birth Center: NRP Certification or completed within one year of hire.
Preferred Education: Bachelor's of Science Nursing (BSN). Certification in specialty area (CCRN, CRN, CNOR, CEN, etc).
Preferred Experience: Recent hospital experience and/or previous nursing practice experience. 2 years prior RN experience in desired specialty area. Prior experience with electronic patient documentation system. Prior experience with technology common to specialty area (i.e., cardiac monitors, IV pumps). Bilingual.
Physical Requirements: Lift and carry 40-50 lbs. Push/pull +/- 150-200 lbs. (Patients on bed, stretcher). Lateral transfer 150-200 lbs. (ie. Patient). Occasional sitting. Frequent standing, walking. Frequent twisting, bending, reaching, squatting, kneeling. Continuous keyboard use. Continuous talking, hearing.
Other Requirements: Proficient in English. Verbal and written communication skills. Computer literate: Proficient with Microsoft Office. Ability to multi-task and tolerate high levels of mental stress/strain.
