Position Description: Licensed Practical Nurse Reports To: COO/CMO
FLSA Status: Hourly – Non-Exempt Direct Reports: Medical Assistants for Center
Purpose: At Western Wayne Family Health Centers the Licensed Practical Nurse (LPN) is responsible for both the medical departments. The LPN has a dual role that involves clinical and administrative duties. The individual is responsible for daily management and direction of everything that needs to be completed so that providers and support staff can perform their jobs effectively and patients have the best experience possible. The manager is involved in all the day to say administrative tasks required to keep a clinic site operating smoothly. They must focus on the business aspects of operating a medical office, with duties that include, but are not limited to ordering supplies, hiring staff, managing schedules, responding to patient concerns and /or requests, managing and supervising staff across a minimum of three departments; and problem solving. The three major target goals for daily operations are increasing productivity, providing excellent customer service, and adhering to quality standards in all aspect of operations.
PART I: ESSENTIAL POSITION FUNCTION AND DUTIES
Administrative
- Direct and oversee daily office operations and delegate authority to assigned staff.
- Assist providers and clinical staff in understanding/implementing clinic policies and procedures.
- Establishes and maintains an efficient and responsive patient flow system.
- Responds in appropriate manner to all patient concerns, complaints, and special requests within 24 hours of incident.
- Develops guidelines for prioritizing work activities, evaluating effectiveness, and modifying activities as necessary. Ensures that office is staffed appropriately.
- Oversee activities directly related to providing services and producing daily encounters and coordinating multiple departments to ensure they are all operating under the same direction.
- Implements and upholds established policies, procedures, objectives, quality improvement, safety, environmental and infection control, and codes and requirements of accreditation and regulatory agencies; conferring with executive and management teams and staff members as necessary.
- Work with Quality department to audit and monitor meaningful use measures; HEDIS performance and other quality areas.
- Manage staff, preparing work schedules and assigned specific duties.
- Responsible for planning, recruiting, hiring, orientation, development, and evaluation of clerical and medical support staff.
- Oversees, tracks and places office supply inventory, and ensures offices are opened and closed according to procedures.
- Implement and manage special time-limited projects initiated by internal staff and/or external stakeholders.
- Plan office layouts, expansion and visual displays in coordination with Director of Operations.
- Uses EMR to access and verify necessary information for multiple departments with their respective EMR screens
- Performs other functions as determined by the medical staff.
- Performs other functions as directed by management staff.
Clinical
*Note: Many of these clinical tasks do not require an LPN to administer and may be delegated to a Medical Assistant who must have the competency to conduct the task thoroughly. LPN has the authority to delegate and/or assign tasks to MA staff when necessary and appropriate.
- Provides Physician Support as needed and as a backup to the Medical Assistants
- Responsible for triage functions either in person or by phone
- Care Coordination of patients; either directly or supervising Care Coordinators assigned to this duty.
- Conducts delegated nurse visits and walk-ins including, but not limited to: TB skin test administration and interpretation; operating retinal camera for diabetes retinopathy screening; hypertension follow up; pregnancy testing and counseling; smoking cessation; screening and administering treatment for STDs; anticoagulation management; blood pressure check; Depo-Provera injection; ear lavage; insulin titrations; medication administration; medication reconciliation; newborn screening; suboxone follow up visits; urinary tract infection; vaccine update; ACOGS; Phlebotomy duties..
- Provide patient education on management of chronic illness; medications and treatment; among other topics.
- Facilitate patient self-care management and disease management; especially for those with uncontrolled chronic illness (direct service or supervising Care Coordinators and/or MAs)
- Provides backup support and verification to Lead MA on patient phone calls when needed/required.
- Provides clinical supervision of MAs and Lead MA
