Job description: Title: Michigan Healthy Transitions (MHT) Project Director $60,000 - $70,000 / Posted thru: 9-19-18 Employee Name: TBD Supervisor: Kim Batsche-McKenzie Purpose: To coordinate a grant-funded initiative to provide the Transition to Independence Process (TIP) model in Kalamazoo and Kent counties by collaborating with the Substance Abuse and Mental Health Services Administration (SAMHSA), MDHHS, the Association for Children’s Mental Health (ACMH), the Community Mental Health Services Providers (CMHSPs) in Kalamazoo and Kent counties, Stars Training Academy (TIP model purveyor), the MPHI Evaluation Team and the MHT Leadership Team and stakeholders. Duties and Responsibilities: * Lead and coordinate all activities related to the Michigan Healthy Transitions Project. * Oversee feasibility, readiness, and implementation of TIP training and certification. * Consult in the hiring of local site TIP team staff. * Coordinate communication and consultation with Stars Training Academy for TIP model training, fidelity and ongoing leadership consultation. * Coordinate quality assurance measures within and between TIP sites. * Coordinate evaluation activities and data collection with MPHI Evaluation Team and sites. * Ensure adherence to all county, state and other regulations. * Facilitate communication with external stakeholders. * Coordinate and participate on the Michigan Healthy Transitions Leadership Team. * Develop processes to ensure that federal grant specific reporting is complete and submitted as required. * Attend grantee meetings. Other related duties as assigned. Qualifications/Requirements: Education: Possession of a Master’s degree in, Social Work, Psychology or Human Services related field is required. Experience: Experience with supervision and oversight of an evidence-based practice. Familiarity with Transition to Independence Process Model preferred. Experience providing community-based mental health services to children and their families. Public mental health system experience preferred. Excellent written and oral communication skills. Demonstrated coordination and organizational skills. Important Skills and Characteristics: Knowledge of administrative and operation structure and the impact of that upon project management. Understanding of evidence-based practices including, but not limited to, TIP model. Knowledge of systems change, improvement processes, and outcomes measurement. Ability to travel to sites (Kalamazoo and Kent counties) and to attend grantee meetings in the Washington D.C area. Presentation skills. Ability to communicate effectively in an interdisciplinary setting in order to facilitate problem solving and establish productive working relationships. Ability to function effectively in a fluid, dynamic and rapidly changing environment. Ability to use software applications (i.e. database, spreadsheets, word processing, presentation programs at proficient to advanced level.) Work Environment and Physical Requirements: Job may require moderate physical effort including lifting materials and equipment of RESPONSIBILITY FOR THE WORK OF OTHERS: Lead Worker for the Michigan Healthy Transitions Project: Lead workers act as project leaders predominantly and/or coordinate the work activities of others. IMPACT ON PROJECTS, SERVICES AND OPERATIONS: Project is funded through a competitive federal grant award. Services to transition age youth are a priority area for the public mental health system and this project is expected to inform practice for the system.
