Position Purpose:
Job Description Details:
Utilizes the Nursing Process to perform the following functions:
Complete and document assessments according to scope of practice. Environments of patient care may include patient visits, phone calls, nurse visits and portal inquiries. Develops an individualized plan of care Identify appropriate level of care based on nursing assessment and documented plan of care. Perform nursing protocols (i.e., First OB, wellness visits, interventions, advanced directives, and medication refill as applicable and approved) adhering to organizational procedures.
Coordinate documented plan of care within interdisciplinary health care team and external resources.
Review and revise plan of care based on individual patient response.
Provide patient education and advocacy based on nursing assessment and documented plan of care.
Professional Practice Accountability: Independently pursues opportunities to maintain and improve competence in practice area.
Leadership: Role models positive professional leadership skills and contributes to overall optimal functioning of the office
Performs other related duties as assigned.
