GENERAL SUMMARY:
Under the supervision of the Registered Nurse, performs specific patient care activities and related services necessary to meet the personal needs and provide for patient comfort
PRIMARY RESPONSIBILITIES AND DUTIES:
1. Assists the Nurse in the admission, transfer and discharge of patients by:
(a) completing the patient admission check list and explaining the Nurse Assistant role to patient and family/significant others; and
(b) giving patients a brief orientation to the unit environment and explaining hospital routine;
(c) prepares patients for discharge or transfer patients as directed.
2. Provides personal care to patients when needed in a timely manner as directed by the Nurse including:
(a) giving bed baths, shampoos and sitz baths;
(b) giving shaves, skin care, mouth care and perineal care;
(c) emptying and rinsing ostomy appliances and external urinary devices;
(d) administering enemas;
(e) positioning, turning and ambulating patients;
(f) transferring patients (to bed, cart, wheelchair, etc.) using proper equipment, and proper body mechanics;
(g) applying Posey and soft restraints;
(h) providing assistance with post-mortem care;
(i) preparing patient for meals and assisting with feeding;
(j) providing basic CPR;
(k) oxygen set up; and incentive Spirometer
(l) Attach remote telemetry leads.
3. Ensures patient safety and comfort by:
(a) responding promptly to patient call lights and keeping bedside stocked with appropriate supplies and making "needs list" for supplies required;
(b) reporting observed changes in patient condition and contributing to care plan by communicating information to nurse; and
(c) keeping patients' rooms tidy and changing linens as needed;
(d) provides 1:1 observation of patients as assigned;
(e) participates in determining patient outcomes through the use of the team nursing
model.
4. Obtains and documents patient care information in an accurate and timely manner in the electronic medical record:
(a) T.P.R.
(b) Blood pressure
(c) Height and weight
(d) Intake and output
(e) Stool guaiac
(f)Pulse ox
5. Obtains and/or delivers supplies/specimens as directed.
6. Performs other duties based on unit specific competencies.
BASIC QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. A. Education / Training: High School Diploma or GED equivalent. Must be able to read and write English at a high school level and perform simple arithmetic calculations
B. Work Experience: One to three months' previous experience as a Nurse's Aide; or Successful completion of Certified Nursing Assistant (CNA) training and demonstration of proficiency in all components or enrollment in an RN program with completion of Nursing Fundamentals or basic Nursing class work.
Contacts and Communications: Work requires contact with patients, families, visitors, nursing staff, and other departments in handling a variety of issues. Ability to explain patient care procedures, answer routine questions and calm patient fears, as well as be able to communicate information on patient condition to nursing staff and physicians is required. Requires excellent verbal communication and guest relation skills.
Analytical Demands: Analytical Demands: Ability to discern, analyze, recognize, normal/abnormal patient situations and report findings accurately and timely. Ability to perform various tasks simultaneously and follows directions accurately and in a timely manner. Must have analytical skills for prioritization of departmental and patient needs/requests. Ability to provide a safe environment for patients to prevent injury and/or delay in discharge. Ability to identify emergency situations and initiate safety actions independently.
Computer Skills: Requires keyboard familiarity and ability to effective interface with hospital information and data entry systems as required.
Certification, Licensure, Registration: Basic CPR - Heartsaver.
