Job Summary
Provides personal care and related services to the patient in their residence under the direction, instruction and supervision of the RN.
Duties and Responsibilities
Assist client in maintaining good personal hygiene. Perform personal care and bath as ordered.
Assist in maintaining a healthful, safe environment. Perform household services essential to health care in the home as assigned.
Report changes in patient's condition and needs to the RN or primary clinician.
Assist with ambulating and exercising the client as assigned.
Assist with simple procedures as an extension of therapy services, e.g., range of motion (ROM) exercise as assigned.
Encourage client to become as independent as possible according to the established plan of care.
Complete assignments as instructed by the primary clinicians.
Attend in-service as required by Agency and state and federal regulations.
Confirm the scheduling of visits with Supervisor to coordinate necessary visits with other personnel.
Job Qualifications
Education, Training, and Licensure/Certification
Experience
- Minimum one year as a Certified Nursing Assistant in a hospital, health care facility or home health agency.
Knowledge Skills and Abilities
Ability to read, write and speak fluent English.
Ability to display compassion and sensitivity with vulnerable adults.
Effective oral and written communication skills.
Ability to work independently with minimal supervision.
Ability to perform basic math calculations.
Good communication skills with ability to work compassionately and display sensitivity to persons with varying degrees of physical and/or mental disabilities.
