Job description/Responsibilities:JOB SUMMARY:Within scope of practice and under the direction and supervision of a licensed Registered Nurse or Therapist provides prescribed therapeutic services (including personal care, rehabilitative activities, meal preparation and home making duties) in order to restore and prevent disability for clients, or patients with musculoskeletal, neuromuscular, and cardiopulmonary disorders.SECTION I:PERFORMANCE ACCOUNTABILITYA. KNOWLEDGE OF WORK 1.Assists with activities of daily living and performs simple procedures as directed by the nurse or therapist in the written home health aide care plan.2.Demonstrates active participation in home care department operational functionsB. DUTIES AND RESPONSIBILITIES 1.Performs care per written home health aide plan of care. Care may include:a.Bathing - complete bed bath, partial bath, tub or shower.b.Skin care including back rubs and measures to prevent further breakdown.c.Toileting activities - bedpan, urinal, BSC, or bathroomd.Oral hygiene measurese.Meal planning, preparation and assisting with feeding the patientf.Simple wound care procedures as instructed by the skilled clinician2.Performs simple procedures as an extension of therapy services such as:a.Assist with transfersb.Assist with ambulation independently or with assistive device.c.Performs PROMd.Assists with /verifies completion of exercises per written plan of therapist3.Provides personal care services to patients after reviewing the care plan. Asks questions of the supervisor regarding care to be provided, as needed with the nurse or therapist.4.Performs light housekeeping tasks per care plan.5.Notifies primary clinician & / or supervisor of any change in the patient's condition or if the patients refuse care.6.Accurately obtains and documents vital signs per department policy, if ordered on the care plan.7.Communicates the patient's response to treatment and effectiveness of the plan of care as set by the nurse or therapist.8.Notifies Case Manager or supervisor immediately of significant problems or unusual incidents requiring intervention, and when appropriate, takes action to intervene. 9.Documentation submitted weekly on time.10.All other duties as assigned or required.

Required Qualification:1.Recent experience as a Nursing Assistant in a nursing home or hospital setting or recent experience as a Home Health Aide. Student Nurse with medical-surgical clinical experience.2.High School Diploma or equivalent3.Home Health Aide Certification/Proof of Training required4.Current BLS certification from American Heart Association5.Current driver's license, car insurance, car registration, and reliable automobile required. Ability to travel to:Brighton, Downriver and South Lyon
