Title: Disease Intervention Specialist

Location: Remote/Virtual - Must Live and Work in the State of Michigan
Purpose:The Disease Intervention Specialist's (DIS) role consists of performing public health disease surveillance, case investigation, and linkage to care work concerning the detection and control of hepatitis C virus (HCV) and other communicable diseases. Specifically, the DIS will be responsible for interviewing patients to complete case investigation, provide health education, and link individuals to treatment and supportive services. In addition, this position is responsible for providing navigation services for HCV and other communicable diseases by working with clinical providers, local health jurisdictions, syringe service programs, and private and community-based agencies that serve high-need populations (e.g., persons who inject drugs, perinatal, and justice-involved populations). Should other emerging threats arise, the DIS will be responsible for shifting responsibilities to respond and investigate (e.g., Tuberculosis).
Duties and Responsibilities:
Utilize the Michigan Disease Surveillance System, ACCESS, Redcap, and other records to manage case investigations and data collection.
Review and interpret communicable disease test results.
Initiate prompt communication with patients diagnosed with HCV to complete case investigations and assist with linkage to care and cure.
Educate patients and providers on HCV testing and support follow-up care.
Connect patients to medical care and supportive services to facilitate HCV testing and treatment.
Assist patients with addressing questions and concerns and provide referrals to ancillary services to bridge gaps in care.
Maintain and protect confidential and sensitive health information.
Support Michigan's HCV Elimination strategies and We Treat Hep C initiative.
Attend and complete DIS cross-cutting training.
Respond to infectious disease outbreaks.
Always represent the best interests of MDHHS and MPHI.
All other duties as assigned.
Qualifications/Requirements:
Education: Possessionof a bachelor's degree in a health-related field.
Experience: Minimumof 1 year experience working in a healthcare role or health education role at an agency such as local, state, or federal health department, a community-based organization, or health system is preferred, but not required. Experience working with people living with hepatitis C is highly desired.

Work Environment and Physical Requirements: The position will be remote. Must have reliable home internet connection. A valid vehicle operator's license to attend off-site meetings, conferences, and trainings. Viewing a CRT or VDT screen over 75% of the time.

IMPACT ON PROJECTS, SERVICES AND OPERATIONS: This position has a direct impact on the quality of services provided by MPHI to MDHHS. As part of the Division of Emerging Infectious Diseases, the DIS's primary goal is to help people live longer healthier, and more productive lives by preventing disease transmission by ensuring all people who are affected or have been exposed are promptly tested, linked to care, and treated.
