- Provide the delivery of culturally sensitive team-based care models in delivering expert nursing care to a diverse adult patient population in the Metabolism, Endocrinology and Diabetes (MEND) Clinic and Endocrine Testing Unit at Domino?s Farms.
- Perform telephone triage, including assessment, screening, and clinical advice; direct patient care including injections, patient education, documentation with follow-up, in-clinic hands on procedures, and coordination with ancillary services and related departments, and other duties as assigned.
- Participate as a member of the collaborative health care team, working closely with patients and their families.
- Provide functional supervision to assistive personnel; collaborate with colleagues, other disciplines, and health care/community services to promote continuity of care for optimal patient outcomes.
- Accurate and efficient documentation of nursing care. Independently and completely performs focused assessment to provide best practice patient care for patients and families as appropriate.
- Actively participate and support performance improvement activities related to self, clinic, department, and institutional goals and objectives.
- Knowledge and appropriate use of internal and external resources, including UMH policies and protocols.
