This position is responsible for serving as a telehealth Registered Nurse providing monitoring services to patients enrolled in the Patient Monitoring at Home program. This position is responsible for assessing the daily patient transmissions of physiological data, educating patients on symptom management, triaging patient virtually. The Registered Nurse will also observe, interpret patient symptoms and report to the provider as indicated.
Characteristic Duties and Responsibilities:

- Coordinate with patient, post-acute care provider, case manager, PCP, Specialist and plan of care to reduce ED utilization and acute care hospitalizations
- Monitor daily transmissions of physiological parameters
- Uses the nursing process and clinical judgement, while completing virtual patient assessments, providing patient teaching, based on clinical assessment and patient reported information.
- Accesses trended data to assess patients medical condition and complete a phone visit/in-home visit for further assessment and triage
- Call physicians and other providers at specified intervals
- Follows up with patients when daily transmissions not completed, providing the necessary troubleshooting of equipment issues or addressing patient compliance concerns of daily monitoring.
- Uses multiple computer applications. Documents assessment, education, and interdisciplinary coordination information in the patients EMR.
- Provides patient teaching, and counseling patients and families regarding self-care and evaluates the patients response to care
- Assess patients telephonically and determine the need for medical treatment
- Exercises independent judgment in day-to-day responsibilities, including the establishment of the plan of care and provision of services
- Initiates appropriate referrals to other home health disciplines as necessary and as a multidisciplinary team member, collaborates to coordinate and evaluate care
- Participates in quality initiatives, supporting the remote patient monitoring program, identifying and implementing initiatives to support customer satisfaction and improved patient outcomes
- Evaluates and revises the plan of care based on the initial and ongoing patient assessment, and in partnership with the patient representative (if any) and caregivers, meeting all regulatory time frames.
- Performs and submits timely documentation of nursing assessments, escalation, and interventions in the home according to the plan of care.
- Participates in organization sponsored in-service trainings.

- Current RN license in the state of Michigan
- Two (2) or more years of acute /medical/surgical in the last 5 years
- Previous experience providing triage or virtual care
