GENERAL SUMMARY
The purpose of this job is to provide a comprehensive assessment and evaluation of needs for the older adult population, and adults with disabilities to maintain quality of life while living in the community. Through in-person visits, develops care services, including monitoring, supporting, and advocating for the specific needs of vulnerable adults for access to services: Program; engages in the research of opportunities for community resources, bridging clients' needs with the local community. Serves as a liaison between clients, caregivers, community members, and professionals regarding various medical challenges, aging-related issues, disabilities, and/or resource availability.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Provides comprehensive assessment and analysis of the needs of older adults to enhance the quality of life; provides therapeutic intervention as appropriate to licensure.
- Develops person-centered services, including regular monitoring, support, and advocacy to ensure equitable access to needed services.
- Utilizes technology resources to complete written assessments, progress notes, and service plans in an electronic medical record; communicates with service providers.
- Engages in research regarding professional development, community education, and resource development.
- Provides education and guidance for clients, caregivers, family, and community members regarding areas of trauma, loss, grief, anxiety, depression, cognitive changes, life stressors, chronic pain/illness, co-morbidities, etc.
- Provides evidence-based educational classes for persons experiencing memory loss or dementia.
- Performs related work as required.
