Scope of Position: The PATH Program Coordinator will provide leadership and support to PATH program staff and participants in order to achieve program performance goals and provide program services that enable participants to obtain and retain employment.
Essential Functions:
- Help to ensure that positive outcomes and program goals are met as required by contract
- Supervise activities and conduct performance evaluations for Career Coaches (CCs), administrative and data staff
- Act as the liaison between the program and EDSI administrative staff on issues concerning billing and performance numbers
- Utilize a coaching approach to build collaborative, trusted relationships with participants and staff
- Assist with participant coaching and case management as needed
- Perform case reviews to ensure that required documentation is maintained, CCs are adhering to program policies, and participants are complying with their participation requirements
- Evaluate and approve participant requests for supportive services
- Document all participant contacts in case notes
- Attend partner and provider meetings
- Act as liaison and maintain collaborative relationships with funder and Department of Health and Human Services (DHHS) partners
- Review monthly Work Participation Rate reports
- Maintain a thorough knowledge of PATH program policies and procedures
- Coordinate participant transfers and other case changes with DHHS and other partners
- Coordinate team meetings with PATH program staff to share best practices and implement continuous improvement
