NorthCare Network the Prepaid Inpatient Health Plan serves residents of Michigan’s Upper Peninsula. NorthCare Network is currently recruiting an individual to serve as an Integrated Care Specialist for NorthCare’s Access Department.
This position, under the supervision of the NorthCare Senior Clinical Director, will provide leadership and support for ensuring the coordination and implementation of system change projects across the service system of care. The Integrated Care Specialist will support and participate in community service planning efforts related to those system change projects and provide leadership and consultation to population teams and service providers related to system change and service improvement projects.
RESPONSIBILITIES INCLUDE: will support Integrated Care Team Members in the development and implementation of system change and innovative practices that will improve system and service efficiency and effectiveness. This position will conduct research and assist with the implementation of best practice of Population Health Management, coordinating and providing consultation, training, and assistance to providers on system change and service improvement projects as needed. This person will develop, implement, and monitor system workforce development, assist in writing grant applications and required reporting. Other duties as assigned.
