Job Summary
The Clinician is accountable to the Supervisor for providing clinical services to clients and their families. The clinician is responsible for the assessment, treatment plan, quarterly reviews, and any therapeutic services he/she delivers with a trauma-informed focus. The clinician is responsible for the authorization and reauthorizations for the case.
Duties and Responsibilities
Develop a person-centered treatment plan that focuses on addressing the needs of the client to include family engagement when applicable.
Coordinate care for the client with family and other involved individuals, i.e. school personnel, probation officer, mentor, DHHS, medical personnel, etc.
Provide individual, family therapy, and case management, as needed to each client/family on his/her caseload.
Market and network with community partners and current/potential referral sources. Examples of this include but are not limited to attending community collaborative meetings, distributing marketing material, providing a description of services provided.
Complete all service plans, progress notes, discharge summaries and all other necessary documentation in a timely manner.
Represent Samaritas mission and vision with community partners. Exercise excellence and integrity as the job requirements are fulfilled.
Monitor the quality of overall treatment services to clients on his/her caseload; and evaluate the adequacy of these services to meet treatment objectives.
Participates in regularly scheduled staff meetings, therapist meetings, and any others as deemed necessary by the Leadership team.
Represent Samaritas mission and vision with community partners. Exercise excellence and integrity as the job requirements are fulfilled.
Professional Growth a. Possess a working knowledge of licensing and accreditation standards and work to assure compliance with such standards. b. Utilize educational opportunities and resources offered by the agency and through the profession for continued development of skills and clinical expertise, (conferences, trainings, peer supervision.)
Responsible for timely Quality compliance as related to employee's roles and functions.
Job Qualifications
Education, Training, and Licensure/Certification
Master's Degree in Social Work, Counseling or a related field, from an accredited institution.
Prior experience working with an interdisciplinary team preferred.
Licensed in the State of Michigan (Limited LLMSW, LLPC or Full LMSW, LPC)
Experience
Minimum one year counseling experience preferred.
Minimum one year experience working on a team preferred.
Familiarity with clinical documentation preferred.
