Title: Case Manager - Perinatal Hepatitis B Prevention

Purpose:The Michigan Department of Health and Human Services (MDHHS) Perinatal Hepatitis B Prevention Program (PHBPP)'s mission is to identify hepatitis B surface antigen (HBsAg)-positive women prenatally or at delivery for each pregnancy so that their infants, household and sexual contacts can be tested and treated to prevent the spread of the hepatitis B virus (HBV). This position will work with Division staff, other state or local programs, birthing hospitals and providers to assure that families are identified and provided information to prevent disease complications and ensure completion of medical interventions associated with HBV transmission.
Duties and Responsibilities:
•Establish and maintain relationships with local health departments, maternal support services, providers, labs, hospitals, infant care providers and clients enrolled in program.
•Identify HBsAg-positive pregnant women.
•Identify the infant(s) born to these women and their household and sexual contacts.
•Conduct case management to ensure timely and appropriate prophylaxis is provided to the infant, household and sexual contacts.
•Establish contact via telephone, email or by mail with clients and providers regarding the care and treatment of hepatitis B exposed infants, household and sexual contacts and HBsAg-positive pregnant women.
•Contact clients and agency officials to verify hepatitis B vaccination dates, tests dates, and results.
•Maintain a statewide database of hepatitis B perinatally exposed infants, and household/sexual contacts.
•Enter all updates for vaccine administration, blood test results, demographic information, provider information, and client agency contacts.
•Assure completeness of data entry and data comparison with various programs including the Michigan Care Improvement Registry (MCIR).
•Review the Michigan Disease Surveillance System (MDSS) for completeness of disease reporting.
•Generate progress reports, statistical reports, and annual reports.
•Assist with updating and disseminating program educational materials including program guidelines, Centers for Disease Control and Prevention (CDC) recommendations, and other hepatitis related materials.
•Assist members of the team with surveys, chart reviews, site visits, educational trainings either in person or via webinars, and evaluation of data.
•Assist with conference and educational opportunities, promotion, planning, registration, reservations, scheduling, and distribution of program materials.
•Serve as the contact person for all hepatitis education outreach sessions.
•Assist in conference goals, objectives, agendas, and materials.
•Complete program certificates and correspondence for all attendees.
•Represent the best interest of MDHHS and MPHI at all times.
•Other duties as assigned
Education: Bachelor's Degree in Public Health, Social Work, Health Education or field related to health, medicine or case management.
Experience: A minimum of one to three years of related and progressively more responsible work experience in health or medicine, public health advocacy and education, or case management.
Important Skills and Characteristics:
•Excellent interpersonal and telephone skills.
•Excellent written/oral/presentation communication skills.
•Familiarity with immunization issues.
•Ability to work independently and manage time efficiently.
•Proficient in Word, Excel, Access and PowerPoint.
Work Environment and Physical Requirements: Routine physical activities: Standing, sitting, walking, reaching, and bending. Extensive use of hands while performing data entry, telephone dialing, and typing. Occasionally stooping, lifting, carrying, presentations and travel.
RESPONSIBILITY FOR THE WORK OF OTHERS: No assigned responsibilities.
IMPACT ON PROJECTS, SERVICES AND OPERATIONS: The Hepatitis B Case Manager will be responsible to represent MDHHS in a professional manner. This employee will contact local health departments, private providers, hospital staff and clients daily. The contact will help to assess individual need, provide educational materials, and establish report with different agencies and clientele.
