JOB SUMMARY
As the Primary Care Dietitian, provides nutritional screening, assessment, interventions, monitoring and evaluation to GLBHC patients according to established policies, procedures and guidelines within the scope of practice of a R.D./R.D.N. as established by the Academy of Nutrition and Dietetics (AND). Addresses nutrition related concerns in focus-area standards of practice based on professional performance including, but not limited to, diabetes care, weight management, nutrition support, pediatrics, nephrology nutrition, public health, sports nutrition and community nutrition.
As the WIC Registered Dietitian, is responsible for the case management of high-risk GLBHC WIC clients. Position provides nutritional assessment, nutrition counseling, development of individual nutrition care plans, and monitoring and evaluation for high-risk WIC clients. Prescribes specialized WIC food packages and approves special supplemental foods/formulas. Performs duties of a WIC Competent Professional Authority (CPA) for determining WIC eligibility, nutrition education, breastfeeding support, WIC food packages and referrals.

15% In an individual, group or telehealth setting, provides nutrition assessment, diagnosis(es), intervention and monitoring to primary care patients referred by providers of the health centers. Determines or recommends nutrition prescriptions and initiates interventions based upon the diagnosis(es) established.
10% Counsels WIC clients and their families based on high-risk nutrition related health conditions and other risks identified through the WIC Certification process.
10% Develops individual nutrition care plans with client-centered goals based on screening and assessment for high-risk WIC clients and primary care patients.
10% Develops plan for monitoring and evaluation of an individual nutrition care plan and encourages collaboration of the WIC team to support the WIC client’s continuity of care.
10% Determines WIC applicant’s medical/nutritional eligibility by assessing anthropometric data, hematological data, and health and diet history.
10% Identifies nutrition needs of individual WIC clients and plans and conducts the nutrition education and high-risk components of WIC services.
5% Documents the individual nutrition care plan including: nutrition assessment, intervention, education and follow-up in the WIC record or in GLBHC electronic medical record. When applicable, uses International Dietetics and Nutrition Terminology (IDNT) and the nutrition care process.
5% Motivates WIC clients and primary care patients to make changes and set individual nutrition goals by using Client Centered Services (CCS) and Motivational Interviewing skills during nutrition education and nutrition counseling.
5% Evaluates and determines appropriate WIC food packages, including special supplemental foods/formulas. Approving Class III formulas according to WIC policies.
5% Provides support and promotes breastfeeding to help WIC clients meet breastfeeding goals. If qualified, provides breastfeeding assistance to manage more complex breastfeeding problems.
5% Assesses WIC and primary care client’s need for other health services and makes appropriate referrals.
5% Maintains active participation in the process of reassessing the nutrition needs of the program and the client concerns for the purpose of program evaluation and quality assurance.
5% Documents all work performed as required by WIC and GLBHC policy.

MARGINAL JOB DUTIES
- Answers phones and schedules clients.
- Performs heights, weights, blood pressure, and hemoglobin as needed.
- If bilingual (English/Spanish): translates nutrition education materials and interprets for providers when necessary.
- Performs other duties as assigned by the Nutrition and WIC Services Manager and Primary Care Nutrition Services Coordinator.
