The Community Coach is responsible for the supervision, assistance and training of persons served in the Community Connections Program. Duties include daily implementation of a person-centered plan for assigned individual(s), providing sensory or independent skills training with a community-based focus, assistance with daily living tasks, and community inclusion, as well as other activities that may be stipulated in their plan Activities will be in keeping with the interests, skills and abilities of the persons served, as established in their person-centered plan.
MINIMUM QUALIFICATIONS: Six months volunteer, personal or work experience relevant to the job assignment. Able to communicate effectively in writing and speech, and to complete necessary reports. Able to read and comprehend programs for persons served. Physically capable of providing assistance to persons served with lifting and transferring, and personal care when necessary.
Driving is required for this position, therefore eligible candidates must possess a valid driver’s license, have an acceptable driving record as well as a reliable vehicle, be able to obtain and maintain agency insurance requirements, as well as obtain a chauffeur’s license.
