ROLE OF THE SPEECH AND LANGUAGE PATHOLOGIST (THERAPIST)

The Speech and Language Pathologist (Therapist) evaluates, directs, supervises, and provides speech therapy services to ExpertCare client/consumers as prescribed by the attending physician and authorized by the provider CMH agency. Provides CMH Agency staff, and the patient’s physician with speech therapy consultation, as needed.

ESSENTIAL FUNCTIONS

1. Assesses, plans, and implements rehabilitation services and for speech, language, and/or swallowing disorders.
2. Provides evaluation of speech and language abilities through diagnostic testing. Establishes goals and estimates prognosis for recovering.
3. Records and reports the client’s progress in treatment and any change in client’s condition to the physician.
4. Instructs family member(s) and caregivers, and other members of the client’s health care team in methods of assisting the patient in improving communication skills.
5. Re-evaluates client’s progress and the established rehabilitation program as often as necessary.
6. Prepares periodic reports, as required by the ProviderAgency and ExpertCare, which are sent to the physician and third-party payers regarding care being provided.
7. Participates in in-service, case conferences, and staff meetings as required.
8. Participates in the Annual Person-Centered Planning meeting and Periodic Status Review meetings to provide insight and update on treatment planning and implementation.
9. Submits assessments and progress notes in timely manner, as required.
10. Provides liaison contact and follow-up with other Care disciplines assigned to case.  

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS 

• Primary activities are conducted within a well-lit, climate-controlled environment.
• Trips to client/prospect locations require outdoor travel and potential exposure to unpleasant weather.
• Ability to sit for extended periods of time and possess the normal range of body motion.
• Visual and hearing ability sufficient to comprehend written and verbal communication.
• Ability to deal effectively with stress.
• Must be able to meet the physial demands of providing speech therapy.

QUALIFICATIONS

1. Licensed as a Speech Language Pathologist by the State of Michigan
2. Graduate of program approved by ASHA (American Speech and Hearing Association) Council on Academic Accreditation in Audiology and Speech Language Pathology
3. At least 18 years of age
4. Able to prevent transmission of any communicable disease from self to others in the environment in which they are providing supports
5. Able to communicate expressively and receptively in order to follow individual plan requirements and beneficiary-specific emergency procedures, and report on activities performed.

KNOWLEDGE AND SKILLS

• Must have an understanding of general business and office terminology.
• Must have basic understanding of the MS Office Suite Products/Programs.
• Must have strong interpersonal and communication skills (both verbal and written).
• Must be able to compose routine correspondence and reports.
• Must maintain a valid state driver’s license.
• Ability to plan, organize and prioritize work.
