A Supports Coordinator Assistant, under the general review of various HealthWest supervision, monitors service delivery including completing consumer progress notes for face-to-face meetings with consumers and significant phone or other events; follows up with Person-Centered Planning goals and objectives through appropriate documentation. A Supports Coordinator Assistant completes all documentation required by the agency, Michigan Department of Health and Human Services, Commission on Accreditation of Rehabilitation Facilities and other regulatory bodies as required for clinical staff; coordinates services for consumers appropriate to their wants and needs and the Person-Centered Plan; links consumer to agency and community resources for which they are eligible such as Social Security Insurance, Medicaid, Medicare, Veteran's benefits, pharmacy and others. A Supports Coordinator Assistant advocates for individuals in areas of health and safety, placement, financial, skill-building, social, recreational and family relationships as appropriate and performs other related duties as assigned.
